Dental implantology is a fast-growing area of the modern stomatology. Production of implant systems and prosthetic elements systems is expanding. New concepts and methodologies emerge that serve to increase the success rate of restoration on implants and ensure long-term positive results.

At times of fittings during all clinical stages of restoration on implants, the number of extractions of healing caps or temporary abutments from the gums increases proportionately to the number of implants. Together with this grows the risk of having complications that may lead even to the loss of implants.
Healing caps and abutments (like implants) are made from titanium alloys that are very bio inert and are able to integrate living tissues (in this case – gum mucosa).
Therefore, after installing of healing cap or temporary abutment on the implant, a stable biological connection forms between gum mucosa and the healing cap or abutment. Every time the healing cap or abutment is extracted, this connection is disrupted. This disruption is unavoidably accompanied by a certain trauma. In conditions when fittings of prosthetic structure continue, the risk of contamination inevitably increases, leading to increased risk of mucositis, periimplantitis and later, loss of implants.
Mucositis is inflammation of the mucosa in the area adjacent to implant.
Periimplantitis is inflammation of tissues in the area adjacent to implant, which is accompanied by bone resorption (loss).
All research in studies of the nature of mucositis and periimplantitis is very contradictory. Because of this it is impossible to arrive at any clear conclusion as to how many complications are due to this stage of implantation.
At the same time, there is no doubt left that considerable part of complications arises as a result of contamination during fittings that require frequent extraction of healing caps or temporary abutments during clinical stages of restoration on implants. The endeavor to reduce these extractions to the minimum throughout all the process of restoration on implants, up to its completion, is quite natural, no matter how big is the clinical case or how many fittings need to be done.
The first stage of restoration on implants – taking a cast – requires extracting the healing cap or temporary abutment in any case. This is necessary in order to fix a transfer – an additional element necessary for taking the cast. After that, the healing cap or temporary abutment is replaced. Then in laboratory a working model is made from the cast. The prosthetic structure is made on the working model. Making of the prosthetic structure is a complex and often long process.
The first step in it is making of permanent abutments – mounts for prosthetic structures. They can be made of titanium or zirconium. After making of abutments there are many other laboratory stages of prosthesis, and after each of those, at the clinic during fittings, healing caps or temporary abutments will be extracted from the gum, permanent abutments will be put in their place and base frames of the future prosthetic structure will be fitted on them; then, in subsequent fittings, the frames themselves; then the frames with ceramic or other coatings on them, etc., etc., until full completion of the prosthetic process.
After every fitting PERMANENT ABUTMENTS will be extracted and returned to the lab to continue work. Healing caps or temporary abutments will be replaced into the gum till the next fitting, when all the unpleasant manipulations, connected with increased risk of infection, will be repeated again.
For many years in all aspects of dental implantology there has not been a weaker place than multiple extractions of healing caps or temporary abutments.
These manipulations prejudice long-term positive results of the whole implantation process, including its surgical stage.
Vast number of specialists, though understanding this, had nothing to counter it with for a long time.
Leading companies manufacturing implant systems offer a partial solution by using so-called “abutment replicas” that are basically duplicates of standard abutments. This can only be effective in cases when there is no need for additional custom work and milling of the abutments on models.
But in cases of multiple or total implantation (with maximum number of bearing elements) there is a minimum possibility of doing without custom work. The reasons are: variety of clinical situations, peculiarities of ratio between jaws and desire to achieve high esthetic result.
No doubt, modern scanners and CAD/CAM technologies bring us closer to the possibility of individual manufacture of identical abutments. Nevertheless, the use of these means does not sufficiently solve all existing problems. Besides, not in all regions and not all stomatologists can afford to purchase expensive scanners and their software.
